A 58-year-old female patient received BNT162b2 (BNT162B2), in 2023 as dose 2, single (Batch/Lot number: unknown), in right arm for covid-19 immunisation. The patient's relevant medical history included: "Blood pressure" (ongoing); "Spondylolisthesis of the spine", start date: 2017 (unspecified if ongoing), notes: Had the fusion in 2017, Had since she was a little kid- diagnosed about 20 years ago; "back surgery", start date: 2017 (unspecified if ongoing); "fracture in her back", start date: 2017 (unspecified if ongoing). Concomitant medication(s) included: AMLODIPINE taken for blood pressure abnormal; LEVOTHYROXINE taken for thyroid disorder. Vaccination history included: COVID-19 vaccine (DOSE 1, SINGLE, Dose unknown, injection in the left arm), administration date: 15Mar2021, when the patient was 54-year-old. Therapeutic measures were taken as a result of pain, joint swelling, nerve injury, arthropathy. Clinical Course: Caller is a consumer calling about the Covid shot she had and said that she has developed side effects. She had surgery in 2017 in her back has surgical screws and since receiving the covid shot, she's had nerve damage in her back. She is experiencing nerve damage and loss of balance. She feels like she is slipping on ice and uses a cane. Her ankles swell up and said that she got her first epidural and it helped somewhat. With the nerve damage, she developed a vibration and prior to the epidural she dreaded getting out of a chair and out of bed and said that the pain was so bad. In her iliac spine, it is a nerve damage pain that is like vibration and she can not explain the feeling. Stated that her left foot and her toes are curled and looks like she has been wearing flip flops for 40 years. Stated that the back surgery she had in 2017 was a L4-L5 fusion. Stated that it is affecting her hip joints too especially on the left side. She is left handed and she may favor that side she does not know. This all started after the covid shot. Clarified to be the Covid-19 Vaccine. She has had 2 and has not had any since then. She only saw him once. She also had a doctor that she saw back in Jul2025. She takes pain killers and was taking Advil. She is going to a psychiatrist to get Xanax to keep her calm from the pain and said that the pain will take your breath away. Stated that she can not find a doctor to get here Xanax like she used to take for premenopausal and could not sleep. She gets a trigger behind her eye with the back pain. She has to press on her eye. They did a MRI on her neck and said that they can give her injections in her neck, but she has not had any injections in her neck yet. She has Spondylolisthesis of the spine and they noticed that when she had the fracture in her back in 2017. Nerve damage: started in Summer of 2024. Her last shot she will have to find out, but the first dose was received at a facility due to covid going on. It was a carnival grounds and was given by firemen and paramedics and was ran by the state. Pain: started Summer of 2024. She noticed the loss of balance and felt like she was slipping on ice in Jan 2025. Nerve damage pain that is like vibration: Noticed this in the winter of 2024. Stated that it is a like a vibration that goes through her body like when you have low blood sugar and gets shaky. Stated that it is embarrassing. Makes her feel like she is going to fall. She had the issue with her left foot and her toes are curled that started in the summer of 2024. It started to affect her hip joints in the winter of 2024. She was taking Advil like 12 a day and it affected her stomach and esophagus and never really had issues with indigestion before and said it is better now, but she had a doctor give her pain medication. The trigger behind her eyes has been going on for about 6 months. She is going to see an eye doctor in Apr2026. Got the first dose in Mar 2021 and got the second dose in 2023 in April or May at pharmacy. Covid Vaccine dose: Unknown if the second dose was in the right arm or not. Thinks her second dose may have been in the right arm since she is left handed. She never had covid though. She recently she got a measles vaccine. Received her updated measles injection in Sep 2025. Also recently had a pneumonia and tetanus vaccines. She doesn't take the flu shot because she gets sick from it. Said that she had the flu shot twice in her life. In Oct 2025, nurse Practitioner gave her a cortisone shot and Tramadol and it relieved her pain for a little while. She was dying and could not get a pain medication.
