Symptom narrative

Flulaval- two doses during same flu season; This non-serious case was reported by a other health professional via call center representative and described the occurrence of extra dose administered in a 56-year-old male patient who received Flu Seasonal TIV Quebec (FluLaval 2024-2025 season) (batch number 99H9A, expiry date 30-JUN-2026) for prophylaxis. Concomitant products included INFLUENZA VACCINE INACT SPLIT 3V (FLULAVAL). On 05-MAR-2026, the patient received the 2nd dose of FluLaval 2024-2025 season. On 05-MAR-2026, an unknown time after receiving FluLaval 2024-2025 season, the patient experienced extra dose administered (Verbatim: Flulaval- two doses during same flu season). The outcome of the extra dose administered was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 11-MAR-2026 The medical assistant called to report that a second dose of Flulaval had been administered in error to a patient who had previously received Flulaval. Both doses had been administered during the same flu season. The medical assistant stated that the error had occurred because the order for the patient to receive the vaccine had been a duplicate.
