Symptom narrative

Patient had two separate immunization records listed under two different names. The record showing pending vaccines was reviewed and assessed, and vaccines were administered to address the listed needs. Upon review of the second immunization record, it was determined that the patient was already up to date on vaccinations. As a result, the patient received an additional dose of DTaP (7th dose), Hepatitis A (4th dose), IPV (6th dose), and Varicella (3rd dose).
