Systemic: after receiving vaccine patient reported she had already gotten the flu shot in October 2025-Mild, Additional Details: After receiving the flu shot on 3/7/2026, patient said that she went home to check and her records shows that she got the flu shot at the doctor's office around October 2025. Store immunization history (which supposedly pulls for Immunization Registry - IR) does not show any vaccines and the flu shot was prompted at POS. Insurance also covered the vaccine so there were no indicator to the immunizing pharmacist that patient already received the flu shot for the 2025-2026 flu season
