No other AE; Pharmacist calling with a report of a 16-year-old patient who was inadvertently administered an adult dose of VAQTA. No reportable symptoms. No other AE/No PQC.; This spontaneous report was received from a pharmacist and refers to a 16-year-old patient of unknown gender. The patient's medical history was not reported. The patient's concurrent conditions were not reported. Concomitant therapies were not reported. On 23-Feb-2026, the patient was vaccinated with Hepatitis A Vaccine, Inactivated (VAQTA) suspension for injection (lot #Z006542, expiration date: 20-Oct-2026), 1 mL total (1ml/once) administered as prophylaxis; 16-year-old patient was inadvertently administered an adult dose of Hepatitis A Vaccine, Inactivated (VAQTA) (Accidental overdose). No reportable symptoms. No other adverse events. At the reporting time, the outcome of the event was unknown. The action taken with Hepatitis A Vaccine, Inactivated (VAQTA) was reported as not applicable.
