Symptom narrative

Pt came in for Bexsero (Meningococcal B) and I gave Meningococcal ACWY. Noted during documentation - call father and reported to him. He brought pt back to clinic to receive Men B. I explained my error and apologized profusely. They were both very agreeable. I assured them that the extra dose would not be harmful and should not have any additional side effects.
