Symptom narrative

Patient wanted a flu vaccine and the record in registry did not show flu was administered. During data entry, staff found a duplicate account and a flu was on record. Medication error was filled out. No record of phone # so patient could not be contacted. No adverse reaction while staff was in clinic. Note: clinic was at the jail
