A few minutes after administration of both Cominarty and Varicella vaccine, Patient return back to report itching and twitching to face. Redness was notable to face. injection site is clean, dry intact with no redness or swelling noted. Per Patient, she had no issues with covid vaccines in the past and this was the first time receiving varicella vaccine that she can recall. A rapid response was called in because Patient started complaining of SOB and appears to get anxious. She reports of fasting today and having only coconut water. Water and gram crackers were offered and she was able to eat but started complaining of hard time swallowing. The team assisted her to ER for further evaluation and treatment. Vital sign taken during assessment: BP 138/88 HR 67 RR 18 O2 97%
