Symptom narrative

Warm sensation with tingling and numbness left torso and left leg; weakness right hip, urinary urgency, delayed ejaculation. Given two course of high dose steroids intravenousely without improvement of symptoms although the right hip weakness has gradually improved but still not at baseline.

prostatic adenocarcinoma Gleason grade 6 dysplastic nevi

Other medications

Vitamin D3 1,000 IU daily

Allergies

tape adhesive (redness after 48 hours)

MRI spine: enhancing lesion at level T2 of spinal cord 12/8/25, 12/23/25, 1/29/26 Laboratory results normal except: CSF 2 oligoclonal bands, ceruloplasmin 12 (14-30 mg/dL)
