Symptom narrative

About 30 seconds after HPV vaccine was administered in L deltoid, pt slumped to the ground, had brief LOC about 10-15 seconds, with some generalized symmetric tonic-clonic jerks, and loss of bladder control, followed by immediate awakening to full alertness. Pt declined juice/cookies, declined having vitals taken, just wanted to go home. Left office walking unassisted (with mother nearby), alert and oriented.
