Symptom narrative

Received 4 vaccines as above on 3/12/26. On 3/16/26 he awoke with a red splotchy itchy rash on upper chest. The rash waxed and waned in the same area throughout the day and he sought care around 3 pm. There were no other symptoms. The rash has uriticarial dermatographism characteristics. He began treatment with loratadine 10 mg once daily and hydrocortisone ointment twice daily on 3/16/26. He will be monitored and has been instructed to report to the emergency room if any anaphylactic symptoms and to return to this office if any minor symptoms. he was also scheduled for a follow up visit in 3 days.
