Symptom narrative

Caller stated patient has not reported any symptoms; patient was inadvertently given PEDVAX HIB vaccine to patient concomitantly while receiving third dose of VAXELIS.; This spontaneous report was received from a physician assistant and refers to a 1-year- 7 months old patient of unknown gender. No information regarding the patient's pertinent medical history, drug reactions or allergies, or concomitant therapies was provided. On 27-Jan-2026, patient was inadvertently vaccinated with Haemophilus b Conjugate Vaccine (Meningococcal Protein Conjugate) (PEDVAX HIB) (dose, route of administration and anatomical location not provided, lot number Z007710, which was verified to be valid, expiration date reported as 13-Nov-2027) for prophylaxis and with Diphtheria and Tetanus Toxoids and Acellular Pertussis Vaccine Adsorbed, Inactivated Poliovirus, (VAXELIS), (dose, route of administration anatomical location, lot number and expiration date not provided) (Accidental overdose). Patient had not reported any symptoms (No adverse event).
