Symptom narrative

At 1400 this RN assisted RN with preparation of vaccine for a family of 3, two adults and one child. This RN grabbed vaccine out of refrigerator and freezer for all clients, including the COVID vaccine for child and adult. Even though this RN feels she did check that right vaccine was grabbed for child (6m-11y), it must not have been since lot sticker on sheet matches with COVID lot for adult 12yr+. RN scanned in vaccine to our EHR and did not notice that wrong vaccine was on child's tray. Since this was a private insurance RN also scanned into system which does give warning if vaccine is not appropriate for client and either this system did not warn client or she did not notice the warning and clicked past it. This RN did administer vaccine to child and did not double check at time of administration if vaccine was appropriate. Both this RN and RN do feel that we could have been better about checking that the right dose was being administered before giving vaccine to client. Client was fine after vaccine was administered. No health concerns were reported after vaccination from parents.
