Symptom narrative

1 Abrysvo was delivered on 2/26/26 but was D/Ced on 2/27/26 when another Abrysvo prescription was received. Technician and RPH on duty did not notice that an abrysvo was sent out the day prior and delivered another abrysvo vaccine to the facility on 2/27/26. Nurse at facility had already given a dose on 2/28/26. Another nurse noticed that there was another abrysvo vaccine for the same patient in the fridge and thought the dose wasn't given. That nurse proceeded to give the abrysvo that was found, causing the patient to receive TWO doses of the same vaccine. (vaccine rec of 1 dose/lifetime). Pt stable at time of conversation with nurse that administered 2nd dose. Advised nurse to monitor pt for a couple of weeks for any adversed reactions.
