she is 28 weeks pregnant; No symptoms or side effects reported by patient./No additional AE; she accidentally received a GARDASIL 9 vaccine on 03/03/2026. Patient reported that she was supposed to receive a "TDAP" vaccine; This spontaneous report was received from a pregnant female patient of unknown age and referred to herself. The patient's concurrent condition included immunocompromised. The patient's medical history, past medications and concomitant medications were not reported. On 03-Mar-2026, at the gestational age of 28 weeks (last menstrual period [LMP]: 20-Aug-2025 [not reported but manually calculated], estimated conception date: 03 Sep 2025 and pregnancy due date: 27 May 2026 [calculated with date of LMP]), the patient was accidentally vaccinated by a medical assistant with Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL 9) (0.50 mL) (total) (lot#, expiration date, route of administration, anatomical site and dose number in series were not reported) as prophylaxis (Exposure during pregnancy); she was supposed to receive Diphtheria vaccine toxoid (+) Pertussis vaccine acellular (+) Tetanus vaccine toxoid (TDAP) (Wrong product administered). No information was provided to the patient as to why the Medical Assistant mistakenly administered Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL 9) to her instead of the Diphtheria vaccine toxoid (+) Pertussis vaccine acellular (+) Tetanus vaccine toxoid (TDAP).
