No adverse event; Exposure via direct contact; This spontaneous report was received from a Physician and refers to a female patient of unknown age. The patient's medical history, concurrent condition, concomitant therapies were not reported. On 03-Mar-2026, the patient's child was vaccinated with Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX) (lot #Z013023, expiration date: 12-Aug-2027), administered by Intramuscular. The reporter used the diluent syringe, put a needle on it, turn into liquid, switched the needle before administration so the whole vaccine is in the diluent syringe, she put a new needle. The vaccine was reconstituted with sterile diluent (lot # 2096680, expiration date: 26-DEC-2027). After reconstitution the needle was inserted into the patient's child and popped off during administration. The reporter administered, while in the leg and as slowly pushing the fluid the syringe detached from the needle, needle stayed in patient's child legs and the part of the vaccine itself leaked out. From what she can see the syringe was not cracked or broken, has since discarded the needle and kept the syringe part. The child was partially vaccinated, and it also spilled on the mother (the patient) (Exposure via direct contact). No additional AE or PQC. No one was injured.
