No additional AE; a 19 month old patient received 1 ml dose of VAQTA due to a human error; This spontaneous report was received from a medical assistant and refers to a 19-month-old patient of unknown gender. The patient's concurrent conditions, medical history, previous drug reactions, drug allergies and concomitant therapies were not reported. On 16-Mar-2026, the patient was vaccinated with an adult dose (due to a human error) of Hepatitis A Vaccine, Inactivated Suspension for injection (VAQTA) SYRINGE 1 ml dose, lot # Y017625 has been verified to be valid with expiration date: 11-Jul-2026 (route of administration and site of administration were not provided) given as prophylaxis (Incorrect dose administered). No additional adverse event (AE).
