Symptom narrative

Patient received Moderna Covid vaccine product for 5-11 years, which is 0.25 mL. She should have received a full dose of 0.5 mL. Patient also received only one component of the Penmenvy vaccine, as the MA did not mix it with the MCV powder included in the box. The patient returned to the clinic to receive an additional dose of 0.25 mL Moderna Covid and a dose of MCV MenQuadfi in order to complete her 16-year-old vaccines.
