Patient received two vaccines on 1/23/2026 Abrysvo and Shingrix at 4:11 pm. On 3/17/2026 patient's wife came to the pharmacy asking for detailed information about the vaccine her husband got that day. she stated he died at a parking lot across [withheld store name] 20 minutes after receiving the vaccine. Vaccine info and lot number is provided to the wife.
