Confirmed no symptomatic AE; Inbound call from nurse calling to report TE for PROQUAD that had an excursion on 20-JAN-2026. The caller reports that a dose was administered to patient on 28-JAN-2026. Confirmed no symptomatic AE. Permission to contact HCP granted. No additional; This Spontaneous report was received from Nurse and referred to a patient of unknown age and gender. The patient's medical history, past medications and concomitant medications were not reported. On 28-Jan-2026, the patient was vaccinated with an improper storage dose of Measles, mumps, rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) Lot Number: Z014409; Expiration date: 10-Mar-2027 (dose #, anatomical location and administration route were not reported); diluted with a sterile diluent (lot #, expiration date, anatomical location and administration route were not reported). Administered as prophylaxis. The reporting nurse stated that the Measles, mumps, rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), had an excursion on 20-Jan-2026 and was administered to the patient on 28-Jan-2026. It was confirmed no symptomatic adverse events.
