Symptom narrative

Patient presented to clinic in need of tetanus booster. State vaccine record was printed off, reviewed, Boostrix vaccination was drawn up, and administered to patient. Previous Boostrix vaccine was given less than 2 years ago, this was missed upon first glance at state vaccine receive, thus Boostrix vaccine was given too soon. No adverse effects reported from patient. PCP made aware, advised no hard would come to patient.
