No adverse reaction- Patient rec'd extra dose of flu vaccine- Patient had scheduled a homebound vaccination and had requested High Dose Flu vaccination. High Dose was administered on 3/18/2026. Upon entering dose into State Immunization Database- it was discovered that patient had rec'd Flu vaccine on 9/24/2025. Patient called to make aware. Denies any side effects. Provided direct contact information to report any changes. Counseled to update PCP of extra dose.
