Symptom narrative

No adverse reaction-Patient rec'd extra dose of flu vaccine-patient scheduled homebound vaccination and had requested High Dose Flu. High Dose flu vaccines was administered on 3/18/26. Upon entering dose into State Immunization Database-it was discovered that patient had rec'd flu vaccine on 9/24/25. Patient was called to make aware and denied any side effects. Patient was provided direct contact information to report any changes. Counseled to update PCP of extra dose.
