On an unknown exact day in April 2024, the patient was vaccinated with the first dose of Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL®9); and Hepatitis A Vaccine, Inactivated (reported as: HEP A) (manufacturer unknown); and Hepatitis B Vaccine (Recombinant) (reported as: HEP B) (manufacturer unknown); and Varicella Virus Vaccine Live (Oka-Merck) (reported as: VARICELLA) (manufacturer unknown) reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT), (lot # and expiration date were not reported); and Tdap IPV (Diphtheria vaccine toxoid;Pertussis vaccine acellular;Polio vaccine inact;Tetanus vaccine toxoid) (reported as: POLIO TDAP) (manufacturer unknown); and with Meningococcal vaccine (reported as: MENINGITITS) (manufacturer unknown) (formulation, lot #, expiration date, exact dose, route of administration and anatomical location were not provided) for prophylaxis. On an unknown exact day in April 2024, the patient experienced dizziness, nausea (he did feel dizzy and nauseous back in April 2024) and allergy to vaccine (the provider elected not to give HPV as she thought the patient may be allergic to it based on what was reported and what happened). On an unknown exact day in April 2025, the patient was vaccinated with the second dose of Hepatitis B Vaccine (Recombinant) (reported as: HEP B) (manufacturer unknown) (inappropriate schedule of product administration); and Varicella Virus Vaccine Live (Oka-Merck) (reported as: VARICELLA); and with Ipv (Polio vaccine inact) (reported as: POLIO) (manufacturer unknown) (formulation, lot #, expiration date, exact dose, route of administration and anatomical location were not provided) for prophylaxis. On an unknown exact day in May 2025, the patient was vaccinated with the second dose of Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL®9) (inappropriate schedule of product administration); and with Hepatitis A Vaccine, Inactivated (reported as: HEP A) (manufacturer unknown) (formulation, lot #, expiration date, exact dose, route of administration and anatomical location were not provided) for prophylaxis. On an unknown exact day in May 2025, the patient experienced dizziness and vomiting. The patient felt dizzy and was given a snack and some juice, and eventually vomited and threw up the snack, rested in the clinic for 15 minutes and felt better and went home. Then the patient presented in February 2026 and reported he did feel dizzy and nauseous back in April 2024 and then the patient came the week of 09-Mar-2026 and the provider elected not to give HPV as she thought the patient may be allergic to it based on what was reported and what happened. The Health Care Professional (HCP) is inquiring if this was likely a vasovagal response from the number of vaccines given at one time or was it an allergic reaction to HPV, and should they give the third dose of HPV or not? At the reporting time, the outcome of the events was unknown. The causal relationship between the reported events and the suspect vaccines was unknown/not reported/not provided.
