Symptom narrative

The patient is not experiencing any symptoms; Patient is 13 months old and was inadvertently given GARDASIL 9. Patient was there to get a different vaccine.; This spontaneous report was received from a pharmacist and refers to his daughter a(n) 13-month-old female patient. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 05-Mar-2026, the patient was inadvertently vaccinated with the dose of Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL®9), formulation: injection (lot #, expiration date, exact dose, route of administration and anatomical location were not provided) for prophylaxis. Patient was there to get a different vaccine. Reporter does not know which one, as he was not at the appointment. He believes that it may have been hepatitis a vaccine, inactivated suspension for injection (manufacturer unknown) (reported as: HEPATITIS A vaccine) (product not administered) that she was intended to receive (wrong product administered). The patient is not experiencing any symptoms (no adverse event).
