Symptom narrative

patient was given Vaxelis (DTaP, IPV, HiB, Hep B) and Hib vaccination in same visit. Doctor did not order Vaxelis but instead ordered DTaP solo as they child did not require further vaccination with IPV and Hep B. Doctor was informed of this after patient left and asked to change order to Vaxelis, of which doctor informed nursing staff they gave two doses of Hib. Parents were contacted.
