received only the diluent part of Penmenvy, it was not mixed with the powder; received only the diluent part of Penmenvy, it was not mixed with the powder; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 16-year-old female patient who received Men ABCWY NVS (Penmenvy) (batch number 55ZX7, expiry date 20-NOV-2026) for prophylaxis. On an unknown date, the patient received Penmenvy. On an unknown date, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: received only the diluent part of Penmenvy, it was not mixed with the powder) and inappropriate dose of vaccine administered (Verbatim: received only the diluent part of Penmenvy, it was not mixed with the powder). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable.
