gave an Expired vaccine to patient after doing 2 checks on vaccine with MA and LPN. Called Merck the manufacturer for vaccine and they verified that vaccine was still valid and no need to revaccinate patient unless provider advises something different. Provider notified of error as well as patient's guardian
