Symptom narrative

improperly stored PROQUAD was inadvertently administered to a patient./ temperature 26F for 2 hours and 6 minutes; No additional AE / No symptoms reported.; This spontaneous report was received from medical assistant and referred to a 4-year-old patient of an unknown gender. The patient's concurrent conditions, medical history and concomitant medications were not reported. On an unknown date, the patient was vaccinated with MMR and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) reconstituted with sterile diluent, 0.5 mL, lot number: Z008866, expiration date: 10-Nov-2026 (dose number, route of administration, anatomical location were not provided) for prophylaxis. The vaccine was improperly stored in temperature of 26 Fahrenheit degrees (F) for 2 hours and 6 minutes, and then inadvertently administered on an unknown date to a patient by healthcare professional (Product storage error). There was no previous temperature excursion. No symptoms/ no additional adverse event (AE) reported.
