Symptom narrative

I spoke with the patient who then explained to me that her vaccine was administered too low and "not in her deltoid at all". Honestly her main concern sounded more focused on the efficacy of the vaccine and not as much towards vaccine injury; however, she did state it was "little sore". I apologized on behalf of the pharmacy and told her we would follow up.
