Symptom narrative

Patient had seemed slightly anxious upon entering the room, and had replied "yes" on the Patient Screening Questions that he has "felt dizzy or faint before, during, or after a shot" in the past. TDaP vaccine was administered at 1040, and aproximately 25sec. following, he reported "feeling funny". I moved his chair to be back against the wall, his eyes dilated, and I held his head as it began to drop back. I noted the time. His breathing stopped for less than 10 seconds, and though it appeared as though he might start convulsing as though having a seizure, he immediately "came to", was confused for aproximately 5 seconds, and then became alert and oriented almost immediately after. At 1043, his Sats were 98, pulse was 42. B/P at 1044 was 122/70. I gave the patient an ice pack to place on the back of his neck. Rechecked Sats at 1045, and he was still at 98, pulse now up to 68. Patient rested in the exam room while I charted. He was given water, and then aproximately 30min following the event, chose to leave.
