On Saturday, 03/14/2026, the patient completed the full treatment session and was stable for discharge. Vital signs at that time were: BP 138/84 mmHg (standing), HR 71 bpm, RR 20 breaths/min, and temperature 97.3°F. At approximately 10:52,the patient received 0.5 mL (20 mcg) of the Heplisav-B vaccine administered intramuscularly into the left deltoid by LPN. (Lot #: 947357, Exp:05/07/2028). A few minutes after vaccine administration, the patient reported palpitations and "not feeling well." This CN2 proceeded to assess the patient. Upon evaluation, the patient was diaphoretic, nauseated, complained of abdominal urgency/diarrhea, generalized pruritus, and remained alert and oriented ×4. EMS was activated at 10:57. The patient was administered 50 mg diphenhydramine and placed on supplemental oxygen. Solu-Medrol and epinephrine were prepared for administration; however, staff held administration as EMS arrived promptly and assumed care. At the time EMS arrived, the patient exhibited generalized hives, hypotension, and active vomiting. The patient remained alert and responsive.
