No additional AE; The nurse is reporting that a patient received improperly stored vaccine; This Spontaneous report was received from a Nurse and referred to a patient of unknown age and gender. The patient's medical history was not reported. The patient's past medications were not reported. The patient's concomitant medications were not reported. On 04-Mar-2026, the patient was vaccinated with improperly stored Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live lyophilisate and solvent for solution for injection (PROQUAD) 1 dosage form (valid lot number: Z010730; expiration date: 22-Dec-2026) for prophylaxis (route of administration was not reported) (Product storage error). Temperature excursion was reported as -12.50 C for the time frame of 4 hours 50 minutes 0 seconds (there was no previous temperature excursion). The vaccine was reconstituted with sterile diluent (lot# and expiration date were not reported). It was reported that the diluent did not experience temperature excursion.
