Symptom narrative

The morning after, my son woke up with a crossed eye. We took him to his pediatrician and were referred to [name withheld] Opthamology. Findings included Esotropia, Comitant. Control poor. Other fields of gaze were attempted but unable to be obtained. A sensorimotor exam was needed given the complexity of this patient's strabismus. He has has prescription lenses ever since to try to correct the crossed eye.

GI upset and rash from Augmentin

Sensorimotor Exam 11/19/25 with Dr. [name withheld] of [name withheld] Opthamology. Findings included Esotropia, Comitant. Control poor. Other fields of gaze were attempted but unable to be obtained. A sensorimotor exam was needed given the complexity of this patient's strabismus. The sensorimotor impression is comitant intermittent large angel esotropia
