The patient's medical history was not reported. The patient's past medications were not reported. The patient's concomitant medications were not reported. On 16-Mar-2026, the patient was vaccinated with the first dose of MMR and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) (0.50 mL total / Two dose series, Asked But Unknown Batch/Lot Number) via Intramuscular for Measles, Mumps, Reubela, and Varicella vaccination; reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT) (dose and lot# unknown). On 16-Mar-2026, the patient was vaccinated with a dose of Diphtheria vaccine toxoid;Pertussis vaccine acellular 5-component;Polio vaccine inact 3v (MRC 5);Tetanus vaccine toxoid (QUADRACEL) for prophylaxis. On 18-Mar-2026, the patient experienced full body hives and itchiness (Urticaria). The patient was seen by their pediatrician on the same day and was prescribed ZYRTEC or CALRITIN for the symptoms.
