PT GOT PREGNANT AND REQUESTED TO GET A BOOSTRIX DOSE. PT FORGOT THAT SHE HAD A DOSE IN 08/18/2026 AND RPH FAILED TO CHECK SHOTS RECORD BEFORE GIVING THE VACCINE. PT IS AWARE AND NO SYMPTOMS OR SIDE EFFECTS.
