Symptom narrative

Pt states the day after he received the shingle vaccine 13 March he has 6-7 seizures feverish and dark cloudy urine. Pt states he is fine now. States he does have a seizure disorder. SOP states the seizures were regular seizures not the grand mal he usually has. Pt states he is feeling fine now. Urine is back to normal color. Pt just wanted to let provider know. Pt has no other issues or concerns at this time
