Symptom narrative

As Parent stated, child began to exhibit symptoms 30 minutes after they left the clinic site. Child complained of itchiness which later developed into a rash. Parent stated her rash was accompanied by hives that came in bunches of 3 or four throughout her body. No complaints of swelling or difficulty breathing. Parent was advised to go to Clinic and have child be evaluated, will be contacted again in 3 days time.
