Symptom narrative

Patient signed documents about vaccines and later called as she was reading about the vaccines and stated that she is pregnant. Patient called her OB and went to the ER as the OB could not get her in. Patient reports that ER stated that baby was doing well after ultrasound and recommended routine treatment.

Other medications

Pt got vaccination, and signed VIS form without alerting she was pregnant. Recieved MMR and Varicella. Per pt, medical personal ensured baby was fine and are recommending routine follow up.
