Symptom narrative

Mother states on the day of vaccination the client felt fine. The next day 03/18/2026, client started complaining on soreness at the injection site. On 03/19/2026, swelling was noted around the right auxiliary region. On 03/20/2026, mother states that site was "puffy" and sore. Encourage mother to seek medical care if symptoms were not improving.
