HPI/ROS: Pt is a 12 y.o. male who presents for evaluation of a skin concern involving the left arm that the patient describes as arm swelling . Historian : Self and step-mother . This concern started within the last 2 days. Appearance is red and swollen and is pruritic. Appearance has become larger, more red, and warm to touch over time. Associated symptoms: none. Patient has not had contacts with similar concern. Patient has had new exposures to immunizations . Patient has tried nothing for treatment resulting in no relief. Patient's step-mother states she just noticed about an hour ago that his arm was swollen, warm, and red. She states it feels hard as well. He went to Super Shots and got the meningococcal ACWY and Tdap vaccines (both in the LUE) 3/18/26. Assessment: Injection site reaction, initial encounter Plan: -Apply an ice pack for 10-15 minutes at a time as needed for pain, swelling, or itching -You may use over-the-counter hydrocortisone cream per label instructions as needed for itching/swelling -You may alternate ibuprofen and Tylenol per label instructions as needed for pain/discomfort and swelling (ibuprofen) - Follow up with your primary care doctor in 1 week to discuss clinic visit and any other concerns at that time, sooner if not improving within 24-48 hours or if worsening
