inadvertent administration of only the prefilled syringe of the liquid MenB component of Penmenvy/ inappropriate reconstitution technique; it was only the liquid Men B portion that was administered to the patient; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a 16-year-old male patient who received Men ABCWY NVS (Penmenvy) (batch number CB25N1B) for prophylaxis. On 16-MAR-2026, the patient received Penmenvy. On 16-MAR-2026, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: inadvertent administration of only the prefilled syringe of the liquid MenB component of Penmenvy/ inappropriate reconstitution technique) and inappropriate dose of vaccine administered (Verbatim: it was only the liquid Men B portion that was administered to the patient). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK Receipt Date: 17-MAR-2026 The general response center transferred an immunization program manager who asked for any advice or recommendation about the administration of only the liquid Meningococcal B portion of Penmenvy to a patient. The vaccine administration facility was the same as the primary reporter.
