Symptom narrative

Patient was being seen in office for wellchild visit. Patient was due for Dtap and Hep A vaccines. Patient was incorrectly given Vaxelis (Dtap, Hib, IPV, and Hep B) and Hep A vaccines. Patients parent was notified and physician was notified immediately after vaccine was given. Call then placed to State Vaccine Unit by vaccine coordinator who suggested we call State Public Health Nurse in the area for further guidance. Spoke with Public Health Nurse who advised to call parents (explained that we did this already), and document a vaccine administration error in VAERS.
