From Dr. [name withheld] visit on 3/23/26: Patient into clinic with issues following attempted vaccine administration in the left deltoid. Date of vaccine was 2/13. Failed attempt at vaccination in the left shoulder noted in nursing notes. Patient did not have any pain immediately. Did have significant pain onset approximately 36 hours after the attempted vaccination. Has been using tylenol and ibuprofen since. Was unable to abduct or flex shoulder. In last few days some increased rom however still significant pain. Intermittent pain at night associated with positions or change of position noted as well. No prior shoulder issues. Patient is presenting with symptoms consistent with SIRVA. Instructed on pendulum exercises to avoid frozen shoulder. Referral to physical therapy. Referral to orthopedics, may benefit from injection or imaging
