Symptom narrative

approx 1 min after administration, patient started to not feel well, patient went pale white in the face, clammy, eyes immediately dilated. Patient's body went stiff, he became unconscious and body started shaking. pt was laid on his back initially and then on his side when body started shaking. Within a 30 sec patient became conscious but still clammy and pale. feeling nauseated. reports vison went black. ice pack applied to back of neck, provided crackers and juice. patient was monitored for approx 30 min. pt left in a wheel chair due to the nausea. no vomiting. Vitals within normal limits. MD was notified of the incident. phone call scheduled for later to follow up on how he is feeling.
