No additional AE; one dose of PROQUAD was administered to one patient during a temperature excursion; This spontaneous report was received from an office manager and refers to a patient of unknown age and gender. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 18-Mar-2026, the patient was vaccinated with an improperly stored dose of Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), (lot #Z008223, expiration date: 27-Oct-2026) which was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT), Solution for injection (indication, expiration date, and lot # were not reported). The vaccine underwent a temperature excursion of 14°F to 46°F (23.6°F) for 6 hours. No additional adverse event was reported. Lot # is being requested and will be submitted if received.
