On an unknown date, the patient was vaccinated with 2 doses of measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (MMR II) (strength, specific dosage, dose number, route of administration, anatomical site of application, lot number and expiration date were not provided) with sterile diluent administered for the treatment of Prophylaxis and on an unknown date (reported as about a month ago) received a third dose as a booster dose (Extra dose administered). Pharmacist reported they did not administer this 3rd dose at their facility and did not wish to provide any patient information. No additional AE reported. No PQC reported.
