History taken from the patient. Onset of symptoms last night. States she was given an HPV vaccine (Gardasil 9) at her appointment yesterday morning then last night started experiencing new symptoms. C/o bilateral hands itching, swelling, pain, and tingling. States she had to remove her rings because her hands were swelling so much. Vaccine was administered in her R deltoid. DENIES difficulty breathing, difficulty swallowing, swelling of face/mouth/tongue, or rash. States she takes Xyzal routinely. RN Recommended Disposition: appointment scheduled. Appointment scheduled w/ NP later this morning (provider notified in person). Patient/caller educated about alarm symptoms and instructed to go to ER or call EMS if any develop. Assessment/Plan Allergic reaction T78.40XA Possible allergic reaction but low suspicion for this. Will treat her for possible allergic reaction.
