Symptom narrative

After receiving COVID vaccine patient developed vertigo. She was not sure when exactly she got the vaccine, believes it was in 2021. Patient also believes it was the Moderna brand but was not 100% sure. Patient had to go to the ENT specialist with this issue and then went through balance therapies. Freqeunt falls are still happening due to vertigo.
