incorrect flu vaccine giving to patient. patient received a high dose when she should have received the regular dose. LPN was informed who then informed the provider. the provider called the patient who let her know of the potential side-effects (arm swelling, redness, headache, malaise) patient did not report any side effects at that time (12:06pm)
